Pinpointing those members in need.
Active Disease Management provides ongoing clinical support to "high-risk" members, identified by our Clinical Stratification and Identification (CSID) tool, who have complex and chronic conditions. The Active Disease Management approach monitors an entire client population and focuses on the overall health of the individual member in a comprehensive, holistic manner. Active Disease Management interactively engages the physician, nurse and member in the care process.

Our nurses are supported with unique tools to educate, empower and motivate members to better understand their conditions and take actions to improve their health. Our nurses have the power of the CareEngine® System at their fingertips, and use the ActiveAdvice® workflow tool to create customized care plans and guide their conversations with members. Our nurses obtain and track additional information from members (otherwise unavailable from claims), such as over-the-counter and herbal medications, diet, exercise and allergies. This vital information is then processed through the CareEngine System, which may then issue new Care Considerations.
